In most hospital areas, Phoenix Children’s allows each patient to identify:

Can I walk into Medical Records?

Phoenix Children’s does not recommend relying on a secure HIM office as a routine walk-in destination. Request forms can be obtained at hospital front desks and submitted through the approved release process.
