How long can a record request take?

SSM Health’s authorization form states that a request will be acted on within 30 days. Patients will be notified if access cannot be provided.

Can there be a records fee?

Depending on the request and applicable law, charges can apply for paper copies, electronic media, mailing, supervised inspection or preparation of a summary.

Need actual imaging? Ask specifically for the imaging files/CD/electronic images, not only the written X-ray, CT or MRI report.
